Personal Care Assistance
​
Ensures your individualized plans are met with trained care and comfort in activities of daily living including but not limited to bathing, dressing, eating, positioning, toileting, and behavior monitoring
Individualized Home Supports without Training​
​
Non-medical care, supervision and assistance provided in the home of the person or in the community to achieve increased independence, productivity and inclusion in the community.

Brain Injury (BI), Community Access for Disability Inclusion (CAD), Community Alternative Care (CAC) and Development Disabilities (DD) Waiver
Respite Care​
​
Short-term care services provided to a person when the person's primary caregiver is absent or needs relief.
Available under various waiver programs and basic homemaker services
Individual Community Living Supports (ICLS)
​
ICLS services offer assistance and support for people who need reminders, cues, intermittent/moderate supervision or physical assistance, transportation, and community engagements to remain in their own homes.
Available to people on:
Elderly Waiver (EW) and Alternative Care (AC)
Individualized Home Supports with Training
​
ILS training is direct training from a staff person to address the identified skill development needs of a person in the areas of communication and interpersonal skills, community living, self care, and sensory/motor development.
Available to people on:
Brain Injury (BI), Community Access for Disability Inclusion (CADI), Community Alternative Care (CAC) and Development Disabilities (DD) Waiver
Homemaking

Services that help a person manage general cleaning and household activities such as meal preparation, laundry, shopping, and assistance with activities of daily living (ADLs).
Available under various waiver programs and basic homemaker services
Getting PCA Services
1.
Apply for Medical Assistance
Appy for MA by calling or going to your local county or tribal office.
2.
Get An Assessment
Request a PCA assessment through your county or health plan, which will determine if you qualify for PCA services.
Getting 245D Waiver Services
1.
Obtain a Home and Community Based Services (HCBS) Waiver
Contact your local county or tribal office.
2.
Connect with a social worker
Discuss and select services from your specific HCBS Waiver. Or if you have a care coordinator, inform your care coordinator of your interest to be on the 245D waiver.
